Vital Records Information |
| The Grant County Health district provides birth certificates
for births occurring in Washington State only.
We also provide death certificates for deaths occurring in Grant
County within the last 45 days. The cost for each
certificate is $20.00. Acceptable forms of payment are cash, money
order or credit card. If you are using a credit card, please call
the Moses Lake office at 509-766-7960. There is a $6.00 fee that
is charged for the use of a major credit card. You may also apply
for a birth or death certificate in person at 1038 W. Ivy Ave., Moses
Lake. The fee schedule for vital records is established by the State
of Washington (RCW 70.58.107). |

| The Grant County Health District can only provide
birth certificates for individuals born in Washington State from
1924 to present. You may download a copy of the birth certificate
(EN),
(SP),
or death certificate (EN)
application in PDF format. Certified copies of death certificates
are available locally for approximately 45 days after the event.
It would be best to contact us directly to make sure the document
is still on file locally before ordering. You can access Vitalchek to
order death certificates that are older than 45 days for any county
with a major credit card, or you can contact the State Vital record page directly. |
