We achieve this by connecting them with a wide range of local resources tailored to their unique needs. Our care coordinators play a crucial role in guiding individuals through the complexities of accessing vital support. By facilitating access to these essential services, we aim to enhance overall quality of life and address the social determinants of health that impact well-being. Through our efforts, we strive to create stronger, healthier communities where everyone can thrive.
Care Coordinators can offer the following services:
- Case management
- Connection to community resources
- Assistance with applying for programs
- Assess needs and goals
- Guidance to achieve stability and improved health
